The prospectus and admission form can be obtained from the office of the College -
Original certificate to be deposited to the College at the time of admission for verification only photocopies are submitted.
Last date of submission of form will be notified (if admissions are not closed). Admission Form received after the last date shall not be entertained.
List of candidates for interview will be displayed on notice board of office (if admissions are not closed)/ telephonically informed.
Candidates will be called for an interview before the board (if admissions are not closed) after preliminary screening of application forms.
The Board will look into the candidate's overall personality, presentation, communication skill, intelligent test, general knowledge, knowledge of current affairs, knowledge of basics of medicare, etc.
In case a selected candidate fails to take admission on the last date of admission, her candidature shall not be considered for admission later on. The seats so failing vacant will be allotted to the candidate next in order of merit.
If a candidate is found using any unfair means to influence the Board member(s) or providing false document or statements then, her admission shall be liable to be cancelled without giving any further opportunity.
Eligibility Condition
The candidates must have passed 10+2 from any recognized Board or Council or University or those whose results are awaited but qualify before the date of admission and minimum of 40% at the qualifying examination and English individually.
The candidates from all streams of study are eligible to apply for admission.
The candidates must have attained by 31st December, minimum 17 years of age (no bar for maximum age) for GNM & ANM Diploma course. For this the candidates are required to submit a self attested copy of their class Xerox certificate or any other valid evidence to support the claim.
Students should have u.p. state residential proof or another state.
The candidates should submit medical fitness certificates from chief medical officer at the time of admission which shall be verified by the medical board of the institute.
Male candidate can also apply for the course of GNM, OT AND DMLT.
Admission of students shall be once in a year.
The registration form is required to be duly filled by the candidates in his/her own handwriting.
Evaluation Criteria
Internal Assessment
There Shall be 25% Internal Assessment for all theory papers and 50% Internal assessment for all the practical’s.
A regular and periodic Assessment for each subject and clinical / field experience is to be carried out.
For the purpose of interned assessment there shall be written test in each subject taken by the respective teacher each month.
The student shall be required to maintain the practical record book and report of observation visits and diary for assessment by the teachers concerned, various other techniques for assessment must be used. Marks shall be allotted for each of the following:
Case Study
Case Presentation
Nursing Care plan
Maintenance of record books ( Procedure Book and Midwifery Record Book)
Daily Diary
Area wise Clinical assessment is to be carried out. Minimum two assessment are required in each clinical area
Regular record of theory and practical is to be maintained . Task oriented assessment is to be undertaken. Assessment Shall be maintained by teacher for each student each month. This can be checked by the council / Board. Principal Shall Sign all the records of examination. It Should be displayed on the notice board for the information of the students.
A Candidate must secure 50% marks in internal assessment separately in each theory and practical. To be successful a student must get 50% marks in the internal as well as council or Board Examination of each year.
For a student who appears for any supplementary examination. her/his fresh internal assessment in the failed subject (s) /practical(s) is to be sent to the council /Board.
State Nursing Council /Board should prepare a model perform for performance evaluation for each of the clinical area and circulate to all. Schools of Nursing for maintaining uniformity. Each student is required to maintain the record of following assignment in clinical areas in each year.
